New Patients/Newborns

We are excited that you want to join the Preferred Pediatrics family!

New Patients (We are not accepting new Medicaid patients)

If you are interested in transferring to our practice, please request your prior medical records by completing a medical records request form at your current providers office. You can also complete our medical record release form and we would be happy to fax it to your prior providers office. We are happy to accommodate appointments for your family while we wait for these records to arrive.

You can also complete the new patient paperwork to bring to your first appointment. Please make sure to bring your photo ID and your child's insurance information.

Newborns

As you prepare for your new baby please feel free to give us a call to schedule a tour of our office. We would love to chat with you about our practice.

If you have already chosen us as your pediatrician simply let the hospital know upon admission for delivery. You should expect to see us a few days after discharge for a follow up. You can get ahead on paperwork by printing the new patient paperwork and bring them with you to your first appointment. Also please make sure to bring your photo id and insurance information for your new baby.
